To make it easier for you, you can now send your medical imaging referral straight from your practice management software to Capital Radiology. This quick process is streamlined for practitioners and easy for patients, increasing the likelihood of patients attending their radiology appointment.
There are a number of different ways you can conveniently send your digital patient referral to Capital Radiology.

How to submit your template referral form
Using your PC or phone, complete the editable referral form provided by Capital Radiology. You can download the forms below, and no signature is required.
To set-up e-referrals on your practice management software, please speak to your Capital Radiology Medical Liaison Officer. Alternatively, contact the team on 1300 722 632 or email ereferral@cdn.com.au to organise a time for us to set you up remotely using TeamViewer.
